SCHOLARSHIP ASSISTANCE APPLICATION FORM

The Moody Foundation recognizes that some families may not have the means to cover the cost of extracurricular activities for children. The Moody Foundation has, therefore, established a Scholarship Fund for the purpose of helping to pay these fees for those in need of such assistance.  Applicants must be residents of Midland County. 

 

Terms & Conditions: 

• Proof of annual household income is required by attaching one of the following: A copy of your most recent Federal 1040 tax form or government assistance 1099. Please make sure to include all sources of income, including wages, government or community assistance, child support, as well as any other scholarships. We require that you meet the “low income” standard set by MSHDA. For 2024, Midland County is $52,950 annual household income. 

• Scholarship funds are limited and, therefore, eligibility does not guarantee any dollar amount of funding or even of funding itself. Depending on annual household income, household size, and/or special circumstances, scholarships may cover from 25% - 100% of the requested amount. 

• A Scholarship Committee is responsible for allocating funds in a fair and balanced way that serves the largest possible number of eligible applicants. Limits may be placed on dollar amounts granted per family. 

• Please include with your completed scholarship application:

  • Registration form for the activity

  • Receipts for equipment to show costs supporting your request

  • Proof of income for all those living in your household. 

• These should be mailed or emailed to The Moody Foundation. Please allow up to four weeks to review your application prior to any deadline when the funds are needed.  Please be sure to include your phone number and email so we can contact you. All information you share is kept confidential.
